Primary Care Professionals Instructions:

If in your every day pursuit of caring for cancer patients you find those unable to afford some of the essentials needed for their comfort, CFA wants to help.

If you are affiliated with a non-profit facility, you may call CFA today for direct shipment of products to your agency. (At absolutely no cost) 1-800-578-5284

Hospice/Health Care Organization Instructions:

The very word hospice in today's society indicates care-giving. It's with great pleasure and pride that CFA finds itself in position to assist those that are dedicated to the destitute and sick.

If you are a not-for-profit hospice and find you have need for our services, request an order form today by calling our toll free number, 1-800-578-5284, or you

can complete an Organization Enrollment Form.

CFA's 26,000 square foot Distribution Center is filled with items as shown on our Pictures and Available Products pages .

We update our order forms as products are rotated. Indicate the items you want on the organization order form and either return it to us, if you have a hard copy, or complete and submit the online form. We will send as much of what you request as possible, depending on availability.

All shipping is free of charge.

Home Health Care Professionals Instructions:

For over a quarter of a century CFA has served patients on an individual basis as well as through the hospice/home health care programs in thousands of communities nationwide.

In addition to the items listed on our Available Products pages, our program allows your patient to receive additional items we currently have available. There is never a charge and shipping is paid by CFA.

	Request an application form by calling 1-800-578-5284 , email us at customerservice@cfoa.org, or download an application and mail it to us after completion.
	Return completed application/diagnosis form.
	MSW's Nurses, Home Health & SW's may sign diagnosis form.
	Determine the products needed on application.
	Tell patient to call CFA - ask for current warehouse list.
	Then call monthly for reorders.
